Yale PRIDE Program

Those who live independently at home (not in an extended care facility / nursing home), in one of the communities listed below, and who need assistance getting up or are at risk of falling may participate in PRIDE. There are three ways that you may be contacted to enroll in the PRIDE project:

  1. If you call 9-1-1 for a lift assist in your town, your local first responders should provide you with a brochure explaining the PRIDE Program and ask if you are interested in receiving a call from a PRIDE Staff members. If you agree, one of our staff members will contact you to answer any questions you might have and ask if you would like to participate. If you agree, PRIDE will enroll you and arrange a home visit.
  2. Individuals may also self-enroll if they believe themselves to be at risk of falling in their own home. You may do so by calling 203-785-4676. Our call center staff will answer any questions that you may have about the PRIDE project and will then ask you the best time for a PRIDE Team Member to come back out to your home to enroll you and conduct home visit.
  3. Individuals who are taken to local hospitals after a fall, are treated and released back to their own home, may also be enrolled by one of our trained staff in certain cases. These individuals will be contacted directly when they are in the emergency department.
